HTTP status:400(client)
Problem. The file could not be read as CSV at all. Cause.
Unbalanced quotes or ragged quoting — the CSV reader gave up before any
header was recovered. Fix. Every line must be comma-separated, and a
value containing a comma or a double quote must be wrapped in double
quotes ("" for a literal quote). Re-save from Excel as CSV UTF-8
(Comma delimited) (*.csv) and upload again. Kept separate from
CHW-8321, which means “we read your header and it named the wrong
columns” — that presumes a header was read, so its “rename these columns”
fix would mislead here. HTTP 400.
