Problem. A usage / billing write (assign plan, create plan, update plan, update org_usage_limits) returned 500. Cause. The DB call failed for a non-conflict reason (connection lost, deadlock, schema drift). Fix. Retry shortly; if persistent, check the server logs for the underlying error and contact support. HTTP 500.
