Problem. A required dependency (user store, email sender, TOTP
encryption key) is not wired for this request. Cause. The server
started without its persistent DB, or an operator has not provisioned
the dependency. Fix. Retry shortly; if the error persists, contact
the operator to check the /healthz endpoint for the failing
dependency. HTTP 503.
