Problem. The supplied TOTP or recovery code did not match. Cause. Clock drift, mistyped code, already-consumed recovery code, or the user enrolled a different authenticator. Fix. Check the authenticator app’s current code, retry, or fall back to a recovery code if the device is unavailable. HTTP 401 (or 400 when the field was outright missing from the request).
